Welcome to the College of Central Florida athletic family!
Prior to your academic year starting at the College of Central Florida, your coach will be in touch to outline the athlete medical packet that you will need to complete prior to stepping on campus. Below is the Athlete Medical Packet for you to print out and complete.
In addition to the athlete medical packet, the NJCAA, FCSAA and the College of Central Florida requires that all student-athletes obtain a Pre-Participation Physical Assessment before participation in any team activities. Forms have been provided below for you to obtain a physical (completed by a medical doctor, physician’s assistant, doctor of osteopathy, or nurse practitioner ONLY). If you are unable to obtain a physical prior to attending college, the CF sports medicine team will arrange one for you. However, this may take time and you will not be able to participate until a completed physical is on file.
It is your responsibility to return the forms in a timely manner. You will not be able to participate, or practice, until each of the forms are signed and on file with the College of Central Florida athletic department's sports medicine team.
CF Student-Athlete Forms (required prior to school starting)
In addition to the medical packet and physical form, you will be required to fill out additional forms pertaining to the CF code of conduct, the drug and alcohol policy, as well as a few others. These will be filled out during athlete orientation, but are posted below if you would like to have a copy for your records. Again, you DO NOT have to fill out the forms on your own.
